JOB SUMMARY:

The unit-based Care Coordinator functions as part of an interdisciplinary team of physicians, nurses and other professionals to maintain continuity of patient care for all patient populations from admission to discharge and utilizes appropriate criteria for inpatient care and timely discharges. Responsible for facilitating clinical care guided by individualized patient/family centered plans of care, guidelines and contracts from managed care organizations and other health care insurers, and clinical decisions tools (e.g. clinical pathways, standardized physicians? orders, clinical protocols) and other evidence-based clinical care guidelines. Responsible for optimizing clinical care by mobilizing clinical and community-based resources to maximize efficiency of care and facilitate hospital discharge. Actively participates in designing and implementing institutional program/initiatives to improve patient access to medical center resources, quality of care, cost reduction efforts and patient satisfactions.

MINIMUM QUALIFICATIONS:

Bachelor's degree in Nursing with 3 years related experience required.

License to practice as an RN in the Commonwealth of Pennsylvania.

Experience in case management, care coordination, outcomes management, or discharge planning preferred.
