JOB SUMMARY:

As a member of a multidisciplinary team, in a leadership role, is accountable and responsible for coordinating the care and service of selected patients and for the promotion of collaborative practice and continuity of care. As a result, effective utilization of resour es and the achievement of desired clinical and financial outcomes will be accomplished.

Education:

- Graduate of an accredited school of nursing.

- Bachelor of Science in Nursing is preferred.

Experience:

- At least three years of recent clinical work experience in the specialty area or related clinical specialty in a hospital

setting within the past five years is required.

- Experience with utilization management, discharge planning and/or case management is preferred.

License/Certification:

- Current State Licensure as a Registered Nurse required.

- BLS required.
